Objective:To determine the efficacy and safety of a single dose of the active ingredient orlistat (Xenical®) for the treatment of obesity. Secondary objectives were the incidence of weight loss, body mass index (BMI), waist circumference, and total cholesterol in the first 6 weeks after treatment initiation. This is an open-label, randomized, double-blind, placebo-controlled trial that included 4 phases of treatment. The primary outcome was weight loss or body mass index (BMI) over the first 6 weeks of treatment. Secondary outcomes included incidence of weight loss, body mass index (BMI), waist circumference, and total cholesterol. Patients and investigators were required to complete a standardized questionnaire. Patients were instructed to complete a health assessment form at each time point of the study.

Methods:A questionnaire was developed and distributed to eligible patients to be screened at 6 weeks and compared with those who were excluded. The questionnaire was administered to each eligible patient and evaluated the extent of weight loss or body mass index (BMI) over the 6-week period. The primary outcomes were incidence of weight loss, BMI, waist circumference, and total cholesterol at the 6-week time point. Secondary outcomes included BMI at the 6-week time point, waist circumference, and total cholesterol, along with a patient’s self-rated level of weight change.

Results:Data were collected from 2,049 patients with a mean age of 58.0 ± 14.0 years and 2,049 patients with a mean BMI of 48.7 ± 7.0 kg/m2, and 2,049 patients with a BMI of 48.7 ± 7.0 kg/m2 at baseline. There were 2,049 patients with a BMI of 48.7 ± 7.0 kg/m2 who received the active treatment and 1,012 patients with a BMI of 48.7 ± 7.0 kg/m2 who received placebo. At baseline, the incidence of weight loss was not significantly different between the 2 groups of patients in either of the 3 groups. There was no significant difference in BMI over the 6-week period between the 2 groups of patients. There was a significantly higher incidence of total cholesterol in the active treatment group at baseline (P < 0.05) compared with the placebo group (P < 0.01).

Conclusions:Patients in the active treatment group lost a statistically significant body mass index (BMI) of 46.8 ± 4.6 kg/m2 at baseline compared with 46.7 ± 5.8 kg/m2 at the placebo group. The incidence of weight loss over the 6-week period was significantly higher in the active treatment group compared with the placebo group. The incidence of BMI at the 6-week time point was significantly higher in the active treatment group compared with the placebo group.
